Do you want to start a treatment program with iPractice? Do you already have a referral letter or are you planning to pay for the treatment yourself?
You can apply for an intake and fill out our application form. We will do our best to answer as soon as possible.
Please note: Applications containing a referral letter can be processed before applications without a referral letter. In case you upload your referral letter, please note that the name of the referring doctor should be clearly visible.
If you don’t have a referral letter yet, we recommend you see your general practitioner first.
